Disability Health Network
A disability service provider is someone who provides individual care to people who have a physical or mental impairment. These providers understand the challenges that come with being disabled and can be a shoulder to lean on. They will provide therapy, equipment, and medicine to improve your quality of life. They also strive to gain the trust of their members by involving them in the planning process, ensuring that their needs are addressed, and providing opportunities for socialization. The disability movement has made tremendous progress in recent decades, but despite this, many still face discrimination and low expectations. Changing societal perceptions of disability is a process that requires transformative changes to disability support systems. This transformation requires leadership and collective commitment. We believe that all people deserve to live an active, healthy, and productive life. There is no better time than the present to start transforming these systems and changing lives. The broader goal of the Disability Health Network is to improve health outcomes for people with disabilities and their families. It engages individuals, families, and health professionals, including hospitals, services, and the WA Department of Health. In the framework, individuals with disabilities are represented by general practitioners, health services, and researchers. And with the support of these organizations, the Disability Health Network is well on its way to changing lives.

Local Area Coordination
The new Local Area Coordination (LAC) program for disability service providers in WA is not a perfect model. Some stakeholders are skeptical about the effectiveness of the program, while others are apprehensive. Although the concept behind LAC is a good one, implementation is lagging behind. For example, three stakeholders have expressed concerns about the lack of knowledge and authority of LACs. As an evaluation of the first three years of the programmed, we identified the most important challenges for local area coordination. The biggest concern we identified was the limited impact of the programmed on the number of disabled people. Despite the high level of disability services in Western Bay, only six per cent of people under 65 have availed of support. Nonetheless, these figures show that the organization is making a difference. The evaluation sample included representatives of three priority groups: disabled people, whanau, and service providers. The sample also included people who had previously been interviewed in mid-2012. The second interview with these individuals provided valuable information about the effectiveness of local area coordination. The evaluation sample consisted of people from the Western and Eastern Bays. Some of the participants were Māori. The focus of the evaluation was to determine whether or not local area coordination is beneficial for this population. In addition to the evaluation of the LAC program, the report aims to highlight the challenges and opportunities associated with the program. While the State Government supports the concept of local area coordination, the role of local area coordinators is not always clear to the community and families. To improve local area coordination, a 13-member review steering committee will be set up, led by Dr Philip Deschamps. This committee will include representatives from non- government disability advocacy organizations and disabled personal care perth providers. The study will take four stages. The first phase will examine the role and operations of the LAC network, while the second phase will examine the relationship between the LAC and other stakeholders. In the fourth stage, the review will consider the value of the LAC in the State and its relationship with other stakeholders.

Self-assessment
A self-assessment form is now a part of the new model for disability services in Western Australia. It replaces the traditional needs assessment. The form can be completed at any time and with anyone, and it is then discussed with a NASC (National Association of Service Coordination) to determine the types of disability support that are appropriate for each individual. The form is designed for use in the Bay of Plenty or Lakes regions, and was developed by the Ministry of Health and the New Zealand Needs Assessment and Service Coordination Association. This self-assessment is important for health service providers in many ways. It helps you improve your services by identifying what your service users need. INDIVIDUALIZED FUNDING
The Enhanced Individualized Funding (EIF) program enables eligible people to purchase disability supports directly. This funding is available in Western Australia and the Eastern Bay of Plenty. It provides more control over the budget and allows individuals to choose the types of support that best suit their individual needs. It also allows individuals to use their disability support budget on a variety of services and supports. The EIF program also allows individuals to use their funds to pay for supports other than those provided by other government agencies.
